The 521211002 is a security T-525SW Model Reader Kit from GE Security which has been designed with Wiegand/S-F/2F comm. support as an electrical box mount in a charcoal gray color with pigtails, an MR/J box, and an installation wrench. The 521211002 security reader kit comes with metal and non metal surface installation, a 6-16VDC power input, allows easy migration to credential technologies, features status, alarm, and perimeter tampering tri-state LED/beep indications, and is completed with an internal alarm alerting function that alerts security staff if a perimeter or reader is breached. This high grade security reader is compatible with multi-vendor 125kHz Proximity credential technologies such as Corp 1000, Vicinity Smart Card, Mifare, and HID Proximity, and can be easy to use.

I have not hooked it up yet as this is new construction and you cannot have electronics in a building before the drywall goes up, but it was easy to select and arrived on time and when we asked for help to see if we could use RG6, they were helpful. What is not to like about S-V.com :)
